RN Assessment and Evaluation
We have registered nurses who will conduct an assessment and evaluation of your care plan. During the assessment phase, our nurses will take note of any relevant health information that will help them in planning for your care. It is during this time that they will identify the appropriate care services that will cater to your needs.
Later, they will conduct an evaluation to see if there is any improvement to your health. If there aren’t any significant changes, they might assess your needs again and see which area they need to improve on or if there are adjustments to be made.
